Before you apply: To be eligible, you must fall within the following guidelines: -Be homeless or likely to become homeless in the next 30 days

-Be 16-21 years old and living without children. (If you are 16-17 years old, your parent/guardian must sign consent.)

-Apply to the program voluntarily and be willing to participate in a community environment. (You will have roommates.)

-Have no recent violent charges/convictions and no sexual offenses.

-Be drug and alcohol free. You may apply if you are in a treatment program.

-Have the ability to follow rules and make a commitment to the program.

-Be able to live independently and are committed to working toward self-sufficiency.

-Are not at high risk of becoming suicidal.

-If you have a mental health or developmental disability and/or are taking medication, we will need to have a treatment plan, School Individualized Educational Plan (IEP), or some type of documentation from a professional outlining the diagnosis and treatment.

-If Stepping Stones cannot meet relevant needs outlined on document, needs must be able to be met by another agency during your participaiton in the housing program.

Stepping Stones utilizes a federally mandated database for intakes, tracking data, and exits. The guidelines for how this database is utilized and secured for your safety is explained here.
Two Major Rules of the Program: -You must have or work toward obtaining your high school degree or GED.

-You must be able to hold a job (minimum 15 hours/week for students; 40 hours/week if not.)

I hereby release Stepping Stones and all its agents and employees, the law enforcement agency and all employees of law enforcement agencies furnishing information, from all liability resulting from the furnishing of this information to Stepping Stones. I certify that the statements made by me on this form are true, complete, and correct to the best of my knowledge and belief and are made in good faith. I understand that any false statements made herein will void my Application for Residency and any actions based on it.

Once you press submit, the first step of the application process has been completed and your application will go to our Independent Living Coordinator, Elena Larson. If the contact information that you provided changes, please give us a call at 812-339-9771 so we can update your application.

Application/Screening Process

1. Applicant fills out Stepping Stones online intake form, or calls Stepping Stones to initiate the application process.

2. Independent Living Coordinator (ILC) calls Applicant within three days to discuss eligibility and set up an interview. Applicant may be asked to provide psychological, medical, and educational records.

3. If eligible, applicant is interviewed at Stepping Stones by ILC, and completes an Application and Consent for Authorization form. If Applicant is a minor, the legal guardian must sign the Consent form.

4. Application is reviewed and information/references verified.

5. Applicant is notified of admission/waitlist status within one week after the interview.

6. After the interview and notification of admission/waitlist status, it is the responsibility of the Applicant to keep their name on the waitlist by calling Stepping Stones once per week to inform Stepping Stones that the Applicant is still pursuing acceptance into the program.

7. When a spot in the program opens up, Applicant and ILC set move-in date.
